申請人須知 Notes for Applicant

• 填寫此申請表前，請參閱“植物進口證/授權書申領指南”。
  Before filling this application form, please read "Plant Import Licence / Authorization - Guide for Applicant"
• 請儘量以英文填寫此表格。
  It is much appreciated if this form could be completed in English.
• 如資料無誤，本署通常可在接到申請書後兩個工作天發出輸入種植材料授權書。
  If application is found to be in order, Authorization to Import Growing medium will normally be issued after 2 working days from receipt of the application.
• 申請及簽發輸入種植材料授權書均不收費。
  No fee shall be charged for an application and for the issue of Authorization to Import Growing medium.
• 如選擇親身到本署領取輸入種植材料授權書，請聯絡本署職員(電話: (852) 2150 7000)查詢領取時間。申請人可選擇在其提供的電郵地址領取電子版本之輸入種植材料授權書。
  If you opt to collect Authorization to Import Growing medium at our office, please contact our office at (852) 2150 7000 for the time of collection. Applicant may opt to receive electronic copy of Authorization to Import Growing medium at the email address provided by the applicant.
• 為確保網上申請系統的操作效率，申請人須於開始填寫申請表兩小時內完成，逾時系統會自動終止申請程序，已輸入的資料亦不會儲存。
  In order to ensure smooth operation of the Online Application System, applicant is required to complete the application form within 120 minutes, after which the application session will expire and all data entered will not be saved.
• 如有關填寫此申請表的問題，請致電(852) 2150 7000與漁農自然護理署植物及除害劑監理科聯絡。
  For any enquiries on filling in this form, please contact Plant & Pesticides Regulatory Division of Agriculture, Fisheries and Conservation Department at (852) 2150 7000.
• 本人已閱讀並同意上述有關網上申請輸入種植材料授權書的注意事項。
